CCC Plus Waiver

What is the CCC Plus waiver?

 The Commonwealth Coordinated Care (CCC) Plus Waiver [1915 (c)] provides care in the community rather than in a nursing facility (NF) or other specialized care medical facility. This new waiver combines Elderly or Disabled with Consumer Direction (EDCD) and Technology Assisted (TECH) waivers. 


Who is eligible for the CCC Plus Waiver?

  • Individuals who meet criteria for nursing home level of care (functional dependences & medical nursing needs)
  • Individuals who require skilled nursing care and are considered at risk for nursing home placement
  • Individuals from birth onward are eligible for this waiver


What services are covered under the CCC Plus Waiver?

 • Personal Emergency Response System  (PERS), includes Medication Monitoring 

• Respite Services (Agency- and Consumer-  Directed) 

• Services Facilitation 

• Transition Services 

 • Adult Day Health Care

 • Assistive Technology (AT) • Environmental Modification (EM)

 • Personal Care Services (Agency-  and Consumer- Directed)

 • Skilled Private Duty Nursing (RN and  LPN) 


Waiver enrollment process

  • Contact the individual's local Department of Social Services and request a screening for the CCC Plus Waiver.

Building Independence Waiver

What is the Building Independence Waiver?

The Building Independence (BI) Waiver provides services and supports in the areas of community engagement, employment, day services, residential, behavioral, crisis, medical, assistive and Smart Home technology, environmental modifications and more 


Who is eligible for the Building Independence Waiver?

 

  • Individuals age 18 years and older who meet the Definition of Developmental Disability.
  • Individuals who meet the VIDES screening criteria. 
  • Individuals who meet the adult monthly income limit of $2,205.00 and resource limit of $2,000.00.
  • Individuals who qualify for and maintain Medicaid eligibility.

What is the Family & Individual Support Waiver?

The Family and Individual Supports (FI) Waiver provides services and supports in the areas of community engagement, employment, day services, residential, crisis, medical, assistive and Smart Home technology, environmental modifications and more. This waiver serves individuals of any age.


Who is eligible for the Family & Individual Support Waiver?

  • Individuals who meet the definition of a developmental disability
  • Individuals who meet the VIDES screening criteria. 
  • Individuals who meet the adult monthly income limit of $2,205.00
  • Individuals who qualify for and maintain Medicaid eligibility.

Community Living Waiver

What is the Community Living Waiver?

The Community living waiver serves people of any age with the most comprehensive continuum of services available in the developmental disabilities waiver program. It provides supports to those with the most intensive medical and behavioral needs. It replaces the former intellectual disability waiver.


Who is eligible for the Community Living Waiver?

  • Individuals of any age who meet the definition of developmental disability are eligible for the community living waiver.
  • Individuals who meet the VIDES screening criteria. 
  • Individuals who meet the adult monthly income limit of $2,205.00
  • Individuals who qualify for and maintain Medicaid eligibility.
